Service Provider Affiliation

To become a Home and Community Based Services (HCBS) Community Service Provider, an agency must be licensed through the Kansas Department of Aging and Disability Services (KDADS), be enrolled with Kansas Medical Assistance Provider (KMAP), affiliated with the local Community Developmental Disabilities Organization (CDDO), and be contracted with any of the applicable Managed Care Organizations (MCOs).

KDADS Licensing: Information on the Kansas Department for Aging and Disability Services licensing process can be found on the KDADS website. The potential service provider will need to contact Aaron Norris, KDADS Licensing Manager, to initiate the licensing process. Aaron.norris@ks.gov

KMAP enrollment: To bill and receive Medicaid reimbursement for services provided, you must enroll as a Medicaid Provider and be assigned a Kansas Medical Assistance Provider (KMAP) number for HCBS I/DD or CSW waiver services. Without a current agreement, you will be unable to receive payment for services delivered. If you are interested in providing developmental disability services in Douglas or Jefferson Counties, you must affiliate with Cottonwood CDDO to bill Medicaid for those services.  The link below contains information on affiliation requirements.  Please contact Director of CDDO Administration: Lynette Goldizen lgoldizen@cwcddo.org to request an affiliate agreement.
