Service Provider Affiliation

If you are interested in providing developmental disability services in Douglas or Jefferson Counties, you must affiliate with Cottonwood CDDO to bill Medicaid for those services.  The link below contains information on affiliation requirements.  Please contact Director of CDDO Administration: Angela Levy alevy@cwcddo.org to request an affiliate agreement.
