Targeted Case Management
Case Management services are those that assist the individual in gaining access to medical, social, educational, and other needed services.
HCBS Intellectual / Developmental Disability Waiver
The HCBS I/DD waiver (Home and Community Based Services Medicaid Waiver for individuals with Intellectual Disabilities and Developmental Disabilities) is for individuals aged 5 and over who have intellectual disabilities or other developmental disabilities. It is designed to assist Medicaid beneficiaries to live in the community and avoid institutionalization.
HCBS Community Support Waiver
The Community Support Waiver (CSW) will help Kansans with intellectual and developmental disabilities (I/DD) live independent lives at home and in the community. People with I/DD who do not need continuous, 24-hour support can choose from a range of services that fit their assessed needs.
Choosing or Changing Service Providers
If you are choosing a provider (for TCM, Day, or Residential) for the first time or are interested in changing existing providers you may do so at any time. In order to begin this process please contact Sarah Elliott with the CDDO at (785) 840-1604. The CDDO is responsible for initiating the paperwork for all service provider choices (either initial choices or changes in provider). The CDDO will also be able to provide you with information on how to select the appropriate service provider as well as the availability status of all service providers in our CDDO area.
