• Assistive Technology An item, a piece of equipment, or a product system used to maintain or improve a participant’s functional capabilities, independence and/or vocational skills.

• Behavior Therapy Using research-based strategies to help improve a participant’s quality of life and reduce challenging behaviors like aggression, destruction, self-injury and/or poor anger management.

• Benefits Planning Helping participants understand how promotions, higher wages, increased hours or other work changes could impact their public benefits.

• Career Exploration and Planning Assisting participants in obtaining or advancing competitive integrated employment (CIE) or self-employment.

• Family and Caregiver Support and Training Providing support and training to family or caregivers who provide unpaid care, companionship or supervision to participants.

• Home and Environmental Modifications Physical modifications to a participant’s home based on an assessment designed to help them function with greater independence in a safer, healthier environment.

• Home Telehealth Remote monitoring systems that enable the participant to effectively manage one or more disease(s) and catch early signs of trouble so intervention can occur before the their health declines.

• Individual-Directed Goods and Services Services, equipment, and/or supplies that cannot be obtained through any other CSW service and that address a participant need; approved on a case-by-case basis.

• Individual Employment Supports Providing job-related support for an individual to help them maintain CIE, customized employment or self-employment.

• Life Skills Developing the practical, social, and emotional skills necessary for successful independent living; includes instruction and skill-building. (Service is eligible for self-direction)

• Medication Reminders Providing a scheduled reminder to a participant when it is time to take medications (e.g., phone call, recording, alarm or dispenser).

• Non-Medical Transportation Transporting a participant to waiver or other community services, activities and/or resources, as specified in the participant’s person-centered service plan.

• Occupational Therapy Helping participants achieve greater independence by gaining and retaining the physical, perceptual and cognitive skills needed to perform activities of daily living.

• Personal Care Services Helping waiver participants accomplish tasks that they would normally do for themselves if they did not have a disability, including activities of daily living, health maintenance activities and instrumental activities of daily living.

• Personal Emergency Response System Electronic devices programmed to signal a response center when activated so that participants can secure help in an emergency; includes the rental of a device and installation (when needed).

• Physical Therapy Helping individuals with their motor functioning and mobility; participants receive treatment to help them move and perform functional activities in their daily lives, prevent mobility loss and achieve a healthy and active lifestyle.

• Remote Support Monitoring and assistance from another location in place of physical staff presence; allows the participant to communicate on demand in real time with provider staff who can respond to their needs remotely.

• Respite Providing temporary direct care and supervision for a participant because of the absence of—or need to relieve—unpaid caregivers who normally provide care for the participant.

• Speech and Language Therapy Treating speech and/or language disorders, such as problems with producing sounds and difficulty understanding or putting words together to communicate ideas. (Service is eligible for self-direction)

• Specialized Medical Equipment and Supplies Items that provide a direct medical benefit or remedy to a participant.

• Vehicle Modification adaptations or alterations to a vehicle that is the participant’s primary means of transportation. Vehicle modifications are specified by the person-centered service plan and are designed to accommodate the needs of the participant and enable the participant to integrate more fully into the community and to ensure the health, welfare and safety and integration by removing barriers to transportation.
