HCBS DD Waiver Service Options <\/h3>\n\n\n\n

The HCBS DD waiver (Home and Community Based Services Medicaid Waiver for individuals with Intellectual Disabilities and Developmental Disabilities) is for individuals aged 5 and over who have intellectual disabilities or other developmental disabilities.  It is designed to assist Medicaid beneficiaries to live in the community and avoid institutionalization.  Click below to view a list of services providers affiliated through the Cottonwood CDDO.  <\/p>\n\n\n\n

Choosing or Changing Service Providers<\/h4>\n\n\n\n

If you are choosing a provider (for TCM, Day, or Residential) for the first time or are interested in changing existing providers you may do so at any time.\u00a0 In order to begin this process please contact Sarah Elliott<\/a> with the CDDO at (785) 840-1604.\u00a0The CDDO is responsible for initiating the paperwork for all service provider choices (either initial choices or changes in provider).\u00a0The CDDO will also be able to provide you with information on how to select the appropriate service provider as well as the availability status of all service providers in our CDDO area.<\/p>\n\n\n\n
